Long Term Disease Management

We offer an annual review with the nurse practitioner or practice nurse for all patients with long term conditions. This is an opportunity to ensure that the condition is managed appropriately and to discuss any concerns that you may have about your condition.

You will be invited by letter on or around your birthday. Some patients will also require blood tests before attending for annual review. Time taken may vary according to condition(s) but usually the appointment lasts from 15 to 45 minutes.

Anti-Coagulation Monitoring

The practice offers a monitoring service for patient taking warfarin.

The Surgery staff will ring you between 5.30pm and 6.00pm with the results of your INR test. It is very important that we give you your results. If we cant contact you for any reason you must contact us.

Asthma

The asthma review is for patients who are using inhalers to control asthma. The aim of the review is to achieve and maintain good asthma control.

Any problems or concerns will be addressed, height and weight will be measured, peak flow respiratory rate will be measured, asthma medication and compliance will be reviewed, inhaler technique will be assessed, an asthma management plan will be given and guidance will be given regarding lifestyle.

COPD Clinic

The COPD review is for all patients with various stages of COPD. The aim of the review is to offer support and guidance in achieving and maintaining good quality of life and avoiding exacerbations.

Any problems or concerns will be addressed, lung function tested, level of breathlessness assessed, statistics measured such as weight and oxygen saturation, medication will be reviewed, inhaler technique assessed and guidance will be given regarding lifestyle.

Coronary Heart Disease

The coronary heart disease annual review is for patients who have had a heart attack, angina, coronary stent, bypass surgery, narrowing of arteries in the leg, stroke or transient ischaemic attack.

The aim of this review is to the offer support and guidance to enable you to manage your condition and to prevent further incidents from occurring.

Any problems or concerns will be addressed, statistics such as weight and blood pressure will be measured, blood results will be explained, medication will be discussed and guidance will be given regarding lifestyle.

Diabetic Care

The diabetes annual review is for patients with all types of diabetes mellitus but management of some patients will be shared between the Diabetes Centre at the hospital and here.

The aim of the review is to assist you in achieving and maintaining good control of blood sugar to avoid the complications that sometime occur when control is consistently poor.

Any problems or concerns will be addressed, statistics such as weight and blood pressure will be measured, urine will be tested, feet will be examined, blood results will be explained, medication will be discussed including titration of insulin if appropriate and guidance will be given regarding lifestyle.

Hypertension

The hypertension annual review is for patients treated for high blood pressure. The aim of the review is to maintain good blood pressure control and prevent the problems that sometimes occur with poorly controlled high blood pressure.

Any problems or concerns will be addressed, statistics such as weight and blood pressure will be measured, urine will be tested, blood results will be explained, an assessment will be made of the risk of developing cardiovascular disease, medication will be discussed and guidance will be given regarding lifestyle.

Healthy Heart (CVD)

Patients aged 40 to 74 years and who have no pre-existing conditions such as coronary heart disease, hypertension or diabetes or who are not taking medication for high cholesterol will be invited to attend for a healthy heart check.

The purpose of this is to identify those with a high risk of developing cardiovascular disease (angina, heart attack and stroke) within a 10 year time frame so that interventions can be offered to reduce this risk.

Fasting blood samples will be taken with a follow up appointment with a nurse when statistics will be measured, family health history identified, lifestyle assessed and blood results discussed.

A risk score will be calculated from this information and appropriate interventions offered. Patients identified as high risk will be offered annual reviews whereas and patients identified as moderate or low risk will be offered 5 yearly reviews.

Vaccinations

Vaccines are the most effective way to prevent many infectious diseases.

Travel Vaccinations

Every year, millions of people from the UK travel overseas for both business and leisure. While we enjoy visiting different countries it is easy to forget that many places do not have the same easy access to medical facilities and treatments as the UK.

Before travelling overseas, it is important to be aware of any possible health risks, plus how to prevent problems, or minimise the chances of them occurring. Most importantly, you should know:

  • how to avoid becoming ill when abroad
  • what to do if you become ill when you are abroad, and
  • how to get emergency medical treatment when you are abroad

Immunisation can protect you against becoming infected with a disease. Many infectious diseases found in countries outside the UK can be prevented with immunisation. You can greatly reduce your risk of contracting these diseases by being immunised against them before you travel.

If you are travelling, you can get a travel vaccination form from reception which you will need to fill out for each person that is going away.

The nurse will check over what vaccinations will be required and you will be asked to call in to collect the information and book an appointment for the vaccinations. There may be a charge for some vaccinations – you will be notified of this prior to the appointment taking place.

Childhood Immunisations

Children are given their first immunisation at 8 weeks old by the doctor at the first postnatal appointment.

Clinics for the Immunisations are held on a Wednesday and Thursday afternoon with the nurse, who gives the further 12 and 16 weeks, and again at 12-13 months followed by their last one which is a pre-school booster, given at 3-5 years old.

Influenza Vaccinations

Influenza, commonly known as the flu, is caused by a virus which spreads easily from person to person through infected droplets in the air and by hands carrying the virus.

The flu virus infects your nose, throat and sometimes your lungs. It differs from a cold as symptoms such as fever, sore throat and muscle aches develop suddenly and last about a week. In some cases severe illness and complications such as pneumonia and bronchitis can develop resulting in hospitalisation and/or death.

Flu can also make some underlying medical conditions worse. There is a need to get vaccinated every year because the viruses circulating in the community continually change and immunity from the vaccine does not last a long time.

Am I entitled to a flu vaccination?

You should be entitled to having a flu vaccination if:

  • you’re 65 or over
  • you’re pregnant
  • you have a serious medical condition (click here for further details)
  • you live in a residential or nursing home
  • you’re the main carer for an elderly or disabled person whose welfare may be at risk if you fall ill
  • you or your child (over 6 months) is in a risk group

Yes. Immunity decreases over time and vaccination is needed each year to ensure you continue to be protected. Vaccination is recommended in autumn to allow time for immunity to be strengthened before the flu season starts.
Each year, the flu vaccine will protect against the three strains of flu virus which are most likely to circulate over the winter period. It is important to get it again this year.
Even if you received a flu vaccination towards the end of the last flu season, you should still be vaccinated again before this flu season.

GP Extended Hours

We have now started our GP Extended Hours Service. This will be run on Evenings and Saturday mornings.

The clinics will be held at Bluebell Medical Centre, Trimdon Avenue, Acklam, Middlesbrought, TS5 8SB.

  • We have sub-contacted this service to the ELM Alliance so it will not be a GP from this practice that you see. We will however receive all the information from your consultations.

The Clinics available will be every weekday evening between the hours of 6.30pm – 7pm and every Saturday morning between 10am – 11am. These appointments can only be booked by contacting the surgery. Please do not contact Bluebell Medical Centre to try and book these appointments.

Non-NHS Services

Certain services are performed on a private basis and a fee will be charged for each item. The following are examples.

  • Accident certificates
  • Driving reports and medicals
  • Employment medicals
  • Holiday cancellation insurance forms or fitness to travel
  • Insurance sickness and accident forms
  • Shotgun licence certificates
  • Private sick notes
  • Fitness exam/certificate for regulatory body

Smoking Cessation

Stopping smoking is one of the single most beneficial things that you can do for your health.

We know that quitting smoking isn’t always easy. People who get support from their local stop smoking service find it easier to stop and are four times more likely to stay smokefree.

  • Currently the surgery is unable to offer any smoking cessation clinics however we can point you in the right direction.

Cervical Smears

Women are offered a smear test from the age of 25 years. Women will be sent a reminder when the test is due.

The practice nurses have received special training for this purpose. If you are unable to get an appointment at a convenient time you are able to attend local family planning clinics without an appointment.

Chiropody/Podiatry

You can access this service from a referral from your GP or nurse. Diabetic patients will be automatically referred to this service.

There are regular chiropody/podiatry sessions at the surgery but referral is made by the medical and nursing staff.

Counselling

The counselling service is available to patients aged 16 and over. If you have symptoms of stress, anxiety or depression and would welcome the opportunity to understand and resolve them, counselling could be helpful to you.

To access the service please discuss your request with your GP.

Contraception

All types of contraceptive services are available; those included are The Pill, Contraceptive injections, Coil fitting and Implanon fittings.

Both the doctors and nurses are qualified and able to discuss contraception with you. We also offer counselling for sterilisation and pre-pregnancy advice.

Maternity Care

The Midwifery team offer a full service to pregnant individuals. Your first appointment is usually at eight weeks of your pregnancy; this is your booking appointment and does last a little longer than usual so you will need to make reception team aware that is your first appointment.

  • Ante-natal checks will be made with doctor by appointment.
  • Post-natal examination will also be made by appointment 8 weeks after delivery.
  • Clinics are held on Monday and Thursday afternoons by appointment only.
